Procedure for requesting a doctor of chiropractic transcript:
Parker University Office of the Registrar needs a request in writing. Complete the transcript request form and mail or fax the request to:

Official Transcripts require a $10 processing fee. If you choose to mail the request, the fee can be paid by check or money order. If faxing the request, the fee must be paid by Visa, Mastercard, Discover (include your credit card number with expiration date and billing address on request). We do not accept American Express. If you are requesting a transcript be sent directly to you it will be stamped "Issued to Student." Some schools may accept these but state boards do not accept transcripts stamped "Issued to Student."
